Prevention of wheelchair-related accidents in elderly people in Japan : Based on the different causes of the accidents between Japan and the United States

Objectives : As the spread of mobility and assistive equipment among elderly persons has promoted greater social participation, the number of equipment-related accidents has increased. The most prevalent adaptive equipment for elderly persons is the wheelchair. In this study, we analyzed wheelchair-related accidents in Japan based on difference in causes of such accidents between Japan and the United States and propose procedures to prevent wheelchair accidents. Materials and Methods : A total of 159 cases of wheelchair-related accidents were collected from the database of the Ministry of Economy, Trade and Industry of Japan. Using a SHEL model, we classified them into 4 categories and compared the causes of accidents with those in the United States. The relationship between the safety factors and accident causes were diagrammatically illustrated using a fishbone chart and analyzed. Results : Both in Japan and the United States, falling from a wheelchair was the most frequently observed problem among wheelchair-related accidents. However, in Japan, such accidents often occurred because of the careless conduct of the user or the caregiver, whereas accidents in the United States tended to be the result of a defective wheelchair. Conclusions : Analyzing the causes of accidents of elderly Japanese persons falling from wheelchairs, we found that human factors played an important role. From these results, it is considered important not only to design safer wheelchairs, but also to instruct users in safer methods for operating them to help decrease the human errors causing such accidents.
