The pattern on the individual serum bile acids seems to be characteristic for different types of hepatobiliary diseases. In this study, individual serum bile acids which was analysed using our method reported in part I of this series, in 8 normal subjects and 71 patients with various hepatobiliary diseases. They were mainly classified into two types of disorders, i. e. the first, the case of the liver cell damage such as acute hepatitis, chronic hepatitis and cirrhosis, and second, that of the cholestasis such as intrahepatic cholestasis (IHC) and extrahepatic cholestasis (EHC). In healthy subjects, all components were detected except lithocholic acid. (1) In the case of liver cell damage, conjugated chenodeoxycholic acids were raised. (a) In acute hepatitis, both relative concentrations of chenodexycholic acid and cholic acid were raised to the same extent. And, G:T ratio was lowered associated with recovery of the disease. (b) In chronic hepatitis, conjugated chenodeoxycholic acid was higher in active type than that of inacti ve type. (c) In liver cirrhosis, individual serum bile acids increased significantly as compared with normal subjects, especially increased glycineconjugated chenodeoxycholic acid was noted. Although the C+DC:CDC+LC ratio of liver cirrhosis was able to be distinguished from that of normal subjects, the C:CDC ratio could not. (2) In the case of cholestasis, conjugated cholic acids were raised. Free cholic acid was significantly elevated in patients with intrahepatic cholestasis as compared with extrahepatic cholestasis. These results indicate that the analysis of individual serum bile acids provides the informations for the diagnosis and the pathophysiological state in hepatobiliary diseases.
