The purpose of this study was to assess the platelet function in patients with diabetes mellitus, especially to investigate its relation to diabetic angiopathy. The platelet aggregation induced by two concentrations of ADP was studied in 49 normals and 296 diabetic patients . Observations were made on platelet aggregation with regard to age, duration of disease, mode of treatment, metabolic controle and diabetic complication, i. e., macro-or microangiopathy. There was close relationship between the degree of platelet aggregation and age in not only controle subjects but also diabetic patients without vascular complication, but it was more significant in diabetics than controles. The influence of the mode of treatments, i. e., diet controle, insulin and oral diabetic agent on platelet aggregation was observed. Oral diabetic agent group showed more pronouncedly increased platelet aggregation than the other treatments. Diabetic vascular complications were divided into hypertention and diabetic macro- or microangiopathy, indicating abnormal ECG, nephropathy, neuropathy and retinopathy. These complications have been compared with the degree of platelet abnormality. It was, thus, demonstrated that platelet aggregation was accelerated with age, duration of disease and affected by the mode of treatments, metabolic controle state and diabetic complications. Many investigators have found enl-lanced platelet function in patients with diabetes mellitus and enhanced platelet aggregation was also found even in prediabetic patients. According to these results, increased aggregation was postulated to be a cause rather than a result of diabetic vascular complications. From these results, it was concluded that the measurement of platelet aggregation in patients with diabetes mellitus was of great clinical use for assessing pathophysiologic aspects of this disease, especially about its vascular complication.
