虚血性心疾患に対する A-C bypass 術後の心電図異常、並びに心機能に関する臨床的研究

Electrocardiographic findings and cardiac performances were studied in 52 cases who underwent aortocoronary bypass for ischemic heart disease during past ten years, and following results were obtained. 1) Among 52 patients of A-C bypass, 3 died after operation, and additional one death was found at 6 years after surgery. 2) ST. T changes on ECG appearing after A-C bypass proved no diagnostic value to detect ischemic changes of the myocardium. Abnormal Q wave developed newly after A-C bypass in 3 of 49 survival cases. In these patients, the activity levels of enzyms (CPK, GOT, LDH) showed abnormally high value in comparison with that of cases without new Q wave. 3) Over all patency rate of the bypass graft was 83.8%. The patency rate was higher in the grafts anastomosed to the coronary arteries with the internal diameter more than 1.5 mm, and with bypass flow 40 ml/min. 4) Cardiac performances (CI, EF, LVEDV, Max LV dp/dt) after surgery were improved especially in the group of complete revascularization, and patients of 91.8% returned to full work without any complications. From these favorable results of A-C bypass, it should be recommended for the patients with ischemic heart disease, and with the coronary arteriographic findings suitable to A-C bypass.
