Diuretic and anti-edematous effects of the Kampo medicine Saireito

The Kampo medicine "Saireito" is used clinically in the treatment of various edematous disorders. However, the scientific evidence remains insufficient. The present review describes attempts to clarify the mechanisms behind the diuretic response and the usefulness for suppressing nephrotic edema of Saireito using a basic pharmacological approach. We have demonstrated that Saireito increased urine volume in pentobarbital-anesthetized rats. It was suggested that the diuretic effect of Saireito was mediated by nitric oxide production. Moreover the action of Saireito was probably involved in the inhibition of the anti-diuretic action of vasopressin. Furthermore, it was clarified that Saireito attenuated increase in interstitial fluid volume in anti-GBM nephritic mice. These findings suggest that treatment with Saireito is useful against edema related to nephrotic syndrome.
