顎関節症患者滑液の酸化還元状態に関する研究

顎関節症の発症に,メカニカルストレスや虚血再灌流に起因する活性酸素種(ROS)の関与が示唆されている。そこで顎関節滑液中のROSの存在を調べることにより,顎関節症とROSとの関連性を検討した。顎関節症患者女性14名と健康成人女性8名の顎関節滑液と血清中に存在するアルブミンの酸化還元状態をHPLCで測定した。また,顎関節症患者3名の滑液中のスーパーオキサイド(O_2^-)の有無をpH-jump ESR法で測定した。その際,滑液中のO_2^-の不均化反応を止めるために強アルカリ(5.0M NaOH)を入れて3つに分け,それらを1,15,30分経過後に液体窒素で凍結させて測定試料とした。顎関節滑液中のアルブミンは血清中のそれに比較して酸化型アルブミンの増加がみられた。さらに,患者滑液中にはROSによって生じたと思われる不可逆的な酸化型アルブミンが増加していた。pH-jump ESR法では過酸化水素(H_2O_2)から生じるO_2^-のESRシグナルが確認でき,そのシグナル強度はNaOH混合から凍結までの時間が長くなるに従って大きくなった。HPLCの結果から,顎関節滑液は血清より酸化状態が強く,患者群の関節内はコントロール群よりROS発生が多いことが示唆された。さらにESRの結果から,患者滑液中にはO_2^-が存在することが明らかとなった。これらの結果より,顎関節症の発症に関連してROS産生の密接な関与が強く示唆された。
