We have developed a highly sensitive method for the determination of individual 3 α-hydroxy bile acids in serum using high performance liquid chromatography (HPLC) combined with immobilized enzyme. After automated pretreatment, samples were analyzed on a column (Medipola Bile). The HPLC eluent consists of ammonium acetate solution, metanol and ace. tonitrile. The enzymatic reaction was measured with a fluorescence detector. The linearity of the standard curve, the recovery ratio, the reproducibility and resolution function were satisfactory. Dynamic changes of total serum bile acids (TBA) and individual serum bile acids after an oral ursodeoxycholic acid (UDCA) load were studied in normal subjects and patients with chronic pancreatitis. In normal subjects the TBA peaked 45 min. after the UDCA load. Within 120 min. after the load, TBA rapidly returned to the basal level. Subjects with pancreatitis, however, had significantly lower levels of UDCA 30, 45, 60, and 90 minutes after the load; their TBA and unconjugated UDCA curves were flat. We also studied the dynamic changes in individual serum bile acids after a test meal (egg yolks) in healthy controls and people who had undergone partial gastrectomy. The gastrectomy group had significantly less serum free bile acid at 60 min. than controls. These results indicate that oral UDCA loading tests can evaluate exocrine pacreatic insufficiency in patients with chronic pancreatitis, and that endogenous bile acid loading evaluate the intestinal bacterial overgrowth in patients after partial gastrectomy.
