Quad-helix装置による上顎側方拡大に伴う正中口蓋縫合部と固定歯歯根膜の血管網の変化に関する実験的研究

Quad-helix appliance has been utilized for years in the treat ment of narrow maxillary dental arch and maxillae. It is important to understand the changes of the vascular network in midpalatal suture and periodontal membrane following the maxillary expansion. The reason is because it is critical to maintain the blood supply and circulation for the osteogenesis and bone remodeling following the expansion. The purpose of this study was to examine the morphological changes in the vascular network of the midpalatial suture and anchorage teeth folowing the expansion. Thirty dogs were divided into 6 groups of 5 dogs each, of which one group was used as a control. A quad-helix appliance was installed in the 5 experimental groups. A force of 600g was applied for expansion of the maxillary dental arch. The animals were sacrificed and injected with acrylic resin through the maxillary artery at 3,7,14,21 and 28 day. The thin sections were examined with a light microscope and fine vascular corrosion specimens were examined with a scanning electron microscope. The results were as follows : 1. Midpalatal suture region. l) The amount of maxillary arch width increased from 3 to 14 days after appliance insertion in both anterior and posterior areas of the surviving dogs. 2) The quad-helix appliance caused wedge-shaped expansion of the midpalatal suture, with the anterior area more widely open as shown in the occlusal roentogenogram. 3) 3 to 14 days following the expansion, the many blood vessels increased accompanied by the expansion actively. The new bone formation was observed in parts of the bone wall and surrounding the blood vessels. 4) 21 to 28 days following the expansion, the number of new blood vessels increased significantly and anastomosed. The tube-shaped new bone formation was observed on the bone walls along the stretched fiber bundles and surrounding the blood vessels. 2. Periodontal membrane region. 1) 3 to 14 days following the expansion, the vascular network disappeared and alveolar bone was exposed in correspondence with the hyaline degeneration on the pressure side in the periodontal membrane. On the tension said, the tip of blood vessels formed the loop towards the direction of tension force. 2) 14 to 21 days following the expansion, moth-eaten like undermining bone resorption from both margin and bottom of the exposed alveolar area was obseved on the pressure side. On the tension side, the pumiceous bone apposition was observed actively on the bone wall. 3) 28 days following the expansion, the vascular rearrangement of blood vessels almost finished both sides in the periodontal membrane.
