顎整形力による上顎骨前方歯槽部の後方移動に関する実験的研究 : Corticotomyの効果

Using seven female Macaca fuscata, the effects of corticotomy in combination with the retraction of the maxillary anterior bone segment were evaluated by X-ray graphic cephalometric analysis and dental cast. Furthermore, histopathological changes of the related sutures were examined. Results were as follows : According to analysis of the dental cast, the retraction volume of maxillary anterior bone segment in the experimental group was much larger (average overjet : -3.6 mm, average overbite : -0.3mm) when compared with the control group (average overjet : -0.3 mm, average overbite : -0.4 mm). There was no change of occlusal relationship regarding the upper and lower first molars. The cephalometric analysis revealed that "Point A" and "Metal Implant" on the mid-sagittal site of the upper central incisors in the experimental group showed the dynamic retraction compared with the control group. These results were histopathologically evaluated by changes of the suture structure : Irregularity of functional arrangement of component cells and fibers, an increase of cell component, resorption and formation of bone, and an enlargement of suture space. But these histopathological changes were mainly observed in the compact bone area, and in the spongy bone of both control and experimental specimens a minimal change appeared. At the region of related tooth roots, there was resorption of tooth roots, cellular cementum deposition, irregularity of normal arrangement of control and periodontal membrane, and resorption and deposition of alveolar bone. In general, these histopathological changes appeared remarkably in the control specimens compared with the experimental specimens. These results demonstrated the usefulness of combining a corticotomy procedure with the retraction of the maxillary anterior bone segment by a orthopedic force.
