The purpose of this study is to evaluate the accuracy of the reposition of the maxillary segment performed with the double splint method of the Le Fort I osteotomy. Forty patients, 8 males and 32 females, who underwent bimaxillary surgery (Le Fort I osteotomy and bilateral sagittal splitting ramus osteotomy) performed by the same surgeon, were enrolled in this study. The positions of the incisor and bilateral molar were measured three-dimensionally with the three casts of presurgery, model surgery and postsurgery utilizing the model platform device. The position and actual change were comparatively analyzed with the paired t-test, Wilcoxon signed-ranks test and correlation the results obtained were as follows : 1) There was a significant anterior deviation from the planned position in all cases ; 2) Especially in maxillary prognathic cases, the results revealed a 3.5mm anterior and 1.2mm superior deviation from the planned incisor position ; 3) As for the segmentalized group, the results revealed that the degree of freedom of the maxillary bone segment increased. On the other hand, uniformity of the lateral (left-right) movement of the bone segment was not attained. From these results, accuracy concerned with the reposition of the maxillary segment with the double splint method was clinically acceptable, with the exception of maxillary prognathic cases where posterior repositioning was required. Therefore, the direction of reposition, depending on the presurgical maxillary position, requires much attention and careful planning in orthognathic surgery, including maxillary surgery
