単一歯牙槽部骨切り術におけるBasic Fibroblast Growth Factor(b-FGF)の効果 : 病理組織学的ならびに血管鋳型標本による研究

The healing process after single-tooth dento-osseous osteotomy was observed. In addition the repair mechanism after surgery was studied in cases in which basic fibroblast growth factor (b-FGF) was administered to the osteotomy sites. Thirty-six healthy mongrel adult dogs were used in the experiment. They were divided into 3 groups as follows 1) dogs that underwent single-tooth dento-osseous osteotomy, 2) dogs that underwent osteotomy and fibrin gel was injected at the site, and 3) dogs that underwent surgery and fibrin gel mixed with b-FGF (100±30μg) was injected. The animals were sacrificed using chemicals 3,7 and 14 days after surgery for examination. After completion of the experiment, they were stained with Hematoxylin-Eosin for histopathological examination under a microscope. Moreover, vascular cast specimens were prepared for examination by electron microscopy. Results : On the 3rd postoperative day, a blood clot was found in the osteotomy site, forming granulation tissue in some areas in all 3 groups. A slight difference was detected between the control and fibrin group. Even in the FGF group, formation of young granulation tissue was abundant, although a blood clot was found. On the 7th postoperative day, a blood clot remained in the osteotomy site and young granulation tissue was present in all 3 groups. In the control group, new vessels and granulation tissue were slightly observed. In the fibrin group and the FGF group, new vessels and formation granulation tissue were seen at the osteotomy site. However, slight bony bridging gtarted from the end of osteotomized segments. In the FGF group, new vessels, as well as new bone, was observed most abundantly, and more new bone was observed than in the fibrin group. On the 14th postoperative day, new capillaries, young granulation tissue and new bone bridging were present at the osteotomy site in all 3 groups. The repair mechanism advanced in the order of the control, fibrin and FGF group. Moreover, in the fibrin group and the FGF group, bone resorption was observed at the acute angled site of the edge in the bed bone and osteotomized bone, demonstrating resorption and remodeling of the bone. Conclusion : After single-tooth dento-osseous osteotomy, the healing process advanced more quickly in the FGF group than in the control or fibrin group. These findings suggested the effectiveness of b-FGF in the healing process.
