Orthopedic Radiography played a important role in the conventional X-ray examination. The acceptable image should include good contrast between bony and soft tissue density and better special resolution of surrounding soft tissue structure such as ligaments and fat. The purpose of this study in how to get such a good quality image in the one film by a simple factor. Therefore, it will make ease to identify the abnormality of bony and soft tissue structure. Various intensifier material and technical methods have been emphasize in the journal of radiological Technology since 1960. Our advocated method can meet the recommendation of the Society of Orthopedic Radiology effectively, such as : (1) Using the small focus during radiography. (2) Change the particle size or shape of the intensifier matcrial and layer thickness of the sensitive emulsion on the intensifying screen and X-ray film. (3) Adjust suitable exposure factor. (4) Change the thickness of the part of subject. (5) Using various material and different thickness wedge filter and grid or extension cone to improve density and contrast. We devised five pieces of different thickness of wedge filter made by aluminum-bronze (JIS 6061) and inserted each of these filter according the body status into the tract outside the collimeter of X-ray tube during radiography. The result of this study showed the image contrast and detail of bone structure and surrounding soft tissue got much improved and the KVP can be reduced about 4-10kVp. The absorbed patient dose and surface dose are decreased around 20% less than that of no using wedge filter. Because of the different combination of X-ray film and screen in daily routine work, it is better to use same kind of combination of film and screen along with these five pieces of filter in order to get a good image. In addition, it will be easy to avoid mistake from imaging processing in the dark room.
