Detection of malignant degree in non-papillary dysplastic lesion by Lising DNA instability test

As for histological diagnosis of bladder, it is very difficult to appraise malignant potential of non-papillary dysplastic lesion. The DNA instability test found by Fukuda et al., (1986, 1993) has been proved to be a powerful strategy as a specific marker for the diagnosis of malignancy in oral, stomach, colon and uterus cancers including skin cancer. In the present study, we investigated the above method as a specific malignant marker for dysplastic lesion of bladder making use of immunostaining for other biomarker such as PCNA, p53 and DFF45. Whole bladders with transitional cell carcinoma, biopsy tissue and tissue obtained by transurethral resection of bladder tumor (TUR-Bt) were used. Whole bladders were divided into from 30 to 70 pieces, mapped transitional cell cancer, dysplastic and hyperplastic lesions according to HE staining. Dysplastic lesion was grouped three stages, mild, moderate and severe. All cells oftransitional cell cancer showed a positive reaction for DNA instability test and dysplastic lesion increased positive cells for the test in proportion to its malignancy. Immunoreactions for PCNA, p35 and DFF45 were also showed as same tendency as the DNA instability test. Our results suggest that this test was useful marker for malignant detection of transitional cell carcinoma, non-papillary, non-invasive type grade 1(TCC-NNT-G1) included dysplastic lesion and proved to be superior strategy to diagnose biological malignancy for low differentiating cancer and/or pre-cancer cells in transitional epithelium of bladder.
