乳歯歯髄切断後の創傷治癒に関する病理組織学的研究 : とくに Formocresol 製剤について

The pulpotomy was performed on deciduous teeth and the cases of Formocresol application were divided into groups A, B and C. Histopathological investigations were made of the healing process of the wounds following the pulpotomy. The results were as follows : 1. The necrosed layer observed at the operated site was extensive in group A and relatively slight in groups B and C, as compared with the cases of calcium hydroxide application heretofore reported. 2. On the 3rd day after operation, a calcium phosphate layer showing a positive reaction on von Kossa appeared in group A as the boundary line between the necrosed layer and the normal pulps. The same finding was not observed in groups B and C. 3. From the 14th to 35th days, preodontoblast-like cells such as tall cubic cells, large spindle cells, and polygonal cells were observed directly under the pulpotomy site in group A. They were larger in cell size and had a larger nucleus, as compared with the cells proper to the dental pulps. A formation of a calcified barrier suggestive of an early stage of a dentin barrier was also observed. In groups B and C, inflammatory cells significantly infiltrated into the pulpotomy site. In the dental pulps below, however, fibroblasts appeared significantly and a formation of inflammatory granulation tissue was observed. 4. After 95 days, a formation of a dentin barrier was observed in group A. The cells adjacent to the dentin barrier were recognized, except in part, as odontoblast-like cells. In groups B and C, an accumulated layer of inflammatory cells still remained at the pulpotomy site. However, an apposition of osteodentin from the peripheries was significant. The dental pulps showed a finding of proliferation of the granulation tissue and became fibrous. 5. On the 120th day, a formation of young dentin at the dentin barrier in group A was observed. The structures of dentinal tubules were also observed by gilt staining and Elastica van Gieson staining, and minute reticular fibers were also observed at the barrier. These findings of interest, not reported by previous investigators, were observed in group A. Group B showed findings similar to those on the 95th day. In group C, although internal resorptions were observed in part, inflammatory findings were not observed. In groups B and C, an interesting finding that a continuous apposition of osteodentin from the peripheries to the root canal dentin became more significant. However it did not result in a complete covering and closure of the pulpotomy site. 6. On the basis of the foregoings, group A appears to be useful for clinical application in Pedodontics. Groups B and C, however, seem instable in their effects.
