Though the possible role of renin-angiotensin (R-A) system on the production and maintenance of hypertension has been extensively investigated in experimental and clinical hypertension, the significance of the measurement of plasma renin activity (PRA) is still uncertain. The present investigation was attempted to clarify the direct and indirect roles of R-A system on the pathogenesis of hypertension and to evaluate the clinical significance of the measurement of PRA. Peripheral venous PRA was measured in 200 samples of 118 hypertensive and non-hypertensive patients. PRA was measured by the modification of BOUCHER's method. Twenty milliliter of 10% acetic acid was used, after ammonium acetate solution before elution of the angiotensin fraction from the Dowex resin column. The final residue was dissolved in 0.9% saline to avoid the pressor effect of 20% ethanol. The assay of the pressor activity was performed on female rats, weighing about 150-200g, anesthetized with pentobarbital, atropinized, vagotomized and sensitized by pentolnium. The mean value of peripheral vein PRA in 22 control subjects was 394.8 ±126.3ng per 100ml of plasma on normal unrestricted diet. PRA in 16 cases of juvenile primary hypertension and in 16 cases of uncomplicated essential hypertension was not significantly elevated (476.2 ± 1580ng/ 100ml and 461.1 ± 1231ng/100ml, respectively). PRA was high in 2 cases of malignant hypertension with advanced renal failure. In a case of malignant hypertension PRA was markedly in-creased with the advance to the malignant phase, though it had been normal in the preceding benign phase, and was further increased with the progress of renal failure. Even in malignant hypertension, PRA was not elevated before the advanced renal failure appeared. In 11 cases of chronic glomerulonephritis with renal failure (above 50mg/dl of BUN), PRA was significantly elevated (971.4 ± 248.0ng/100ml), while it was not significantly increased in 9 cases of those without renal failure (481.1 ± 188.0 ng/100ml).
