When compared with general hospital systems, the mental hospital system in Japan is understaffed under the so-called "mental hospital exemption." So the staff cannot secure enough time for service users to have ample counseling opportunities to help achieve self-determination. The quality of service for a large majority of patients is degraded by such structural problems as treatment in closed wards, extremely prolonged period of hospitalization, and a consistently large number of beds per population. On the other hand, since the late 1980's, protection of the human right of the mentally disordered and rehabilitation of mental patients have been taken up and tremendous improvement has been made in terms of important political issue of national and local government agencies. But in reality, not much of an improvement has been noted on the inside of mental hospitals, where characteristics of the asylum have persisted since the Meiji era. What is the root cause of this? This study depicts and analyzes cases within the hospital as well as a case of a mental health and welfare network in the community. Many problems are found in the quality of service that is provided by the first-line professionals in the hospitals and in the community. It contends that the quality of service is the vital element in overcoming stigmatization. It is not permissible to blame only the structural problem of mental health and welfare system as the determining factor in the occurrence of the stigma of the mentally disordered.
