During the outbreak of food-borne disease caused by enterohemorrhagic Escherichia coli (O157) in Sakai, Osaka, 984 patients were treated at Sakai Onshinkai Hospital, a satellite hospital. Twelve patients were underwent appendectomy following diagnosis of acute appendicitis. They were school children who ate polluted school lunch by O157. Bacterial culture tests were performed in 10 of 12 cases. Based on surgical findings and macroscopic observation of them, was diagnosed as phlegmonous in 6 patients and catarrhal in 4 patients. The bacterial culture test revealed O157 in 2 of the 4 patients with localized appendicitis and 3 of 6 patients with appendicitis associated with ileocecitis. This result suggested a relationship between O157 and acute appendicitis. Accordingly, appendectomy is recommended in patients with O-157 infection when acute appendicitis is strongly suspected.
