馬における腺疫腦脊髄炎の 4 例

Recently, the purulent encephalomyelitis in horses as a complication of systemic infections such as strangles have been not infrequently encountered in the field. Because of the occasional outbreaks of the epizootic equine encephalomyelitis, a specific laboratory diagnosis may be required for confirmation of clinical diagnosis between both viral and bacterial encephalomyelitis. There are numerous publications on the microscopical studies of non-suppurative encephalomyelitis contrary to the purulent encephalomyelitis. The authors carried out the clinical and pathological examinations on the brain of 4 cases of purulent encephalomyelitis complicated with strangles comparing with the epizootic encephalomyelitis. The results are summarized as follows; 1) The animals manifested clinically depression, staggering gait, excitement, tetany, visual disturbances, rigidity of the neck and other nervous symptoms in addition to common symptoms in strangles. 2) In the gross and microscopical examination, viscera of the all cases studied except one (No. 2) showed slight septic changes, especially suppurative broncho-pneumonia. 3) In the central nervous system, three cases (Nos. 1,3 and 4) showed purulent meningoencephalomyelitis. In the other (No. 2), pathological changes are restricted in the brain substance and formed an abscess in both cerebral hemisphere respectively. Therefore it was diagnosed as purulent encephalitis (cerebral abscess). 4) In the case, No. 1,purulent meningoencephalomyelitis occurred by direct extension of a metastatic abscess in the frontal region through suppurative periostitis and caries of the bone. In the other three cases, it seems to be that pyogenic organisms gained access to the brain by metastais from suppurative processes in other organs. 5) The histologic differential diagnosis is not diffucult in the changes of the central nervous system between purulent encephalomyelitis caused by strangles and an early stage of Japanese equine encephalomyelitis. In the formers severe polymorphonuclear infiltrations in brain tissues are frequently observed. It is also needed not only cell analysis of the infiltrates, but also systematic and exact examination of the central nervous system.
