<原著>慢性関節リウマチにおける軟骨基質破壊とリンパ球, 滑膜線維芽細胞の相互作用

Irreversible loss of articular cartilage is a natural course of rheumatoid arthritis (RA), but many of the mechanisms underlying cartilage degradation in RA remain to be elucidated. In this study, the lymphocyte-synovial fibroblast interaction on cartilage degradation in RA was examined. The percentage of lymphocytes from peripheral blood, synovial fluid and synovial membrane adhering to articular cartilage that increased in RA patients was compared with that in patients with osteoarthritis (OA). Both in RA and OA, lymphocytes from the synovial membrane showed the strongest adherence to articular cartilage. The adhesion of synovial fibroblasts to articular cartilage increased in RA patients as compared with that in OA patients. In RA patients, lymphocyte-cartilage binding was accerelated by the addition of synovial fibroblasts and synovial fibroblast-cartilage binding was accerelated similary by the addition of lymphocytes. Next, synovial lymphocytes and fibroblasts were analyzed for their ability to stimulate glycosaminoglycan (GAG) depletion from the articular cartilage. The basal level of autolysis was higher in RA cartilage than in OA cartilage. Addition of synovial lymphocytes and fibroblasts increased the GAG release from articular cartilage both in OA and RA patients, and this cell-mediated GAG release was higher in RA than in OA. Both in RA and OA patients, GAG release from articular cartilage was more accerelated by a combination of synovial lymphocytes and fibroblasts than only by synovial lymphocytes or synovial fibroblasts. These findings demonstrated that synovial lymphocytes and fibroblasts are capable of binding to articular cartilage and suggested that during the course of inflammatory synovitis, synovial-lymphocyte-fibroblast interaction may occur in cartilage destruction.
