To investigate morphologically the vascular changes in various lesions, the relationship between cellular characteristics and vascular reaction of lymph nodes was examined in 28 human biopsied cases. The patients were diagnosed as normal lymph nodes and mild lymphadenitis (5 cases), follicular B cell lymphoma (3 cases), diffuse B cell lymphoma (5 cases), immunoblastic lymphadenitis (IBL) and IBL like T cell lymphoma (4 cases), diffuse T cell lymphoma (6 cases), adult T cell leukemia (ATL, 5 cases). Paraffin sections were examined by reconstruction methods of three-dimensional vasculature, measuring number of vascular branchings, thickness of endothelial cells and number of endothelial nuclei for unit length of blood vessels on tissue slides in which endothelial cells were marked out by Factor VIII or UEA-1 lectin. Reticulin fibers of the vascular walls were observed with Galyas's silver stain. Findings by these methods were analyzed in relation to proliferated cellular components of the lymph nodes by immunohistochemical lymphocytes markers. The diffuse B cell lymphoma showed scanty vascular branching, very thin endothelial cells, few numbers of endothelial nuclei for unit length of blood basement membrane, and few reticulin fibers in the walls compared to normal lymph nodes. IBL and IBL like T cell lymphoma showed marked proliferation of vascular branch network and significantly increased number and thickness of endothelial cells with vascular nuclei. Reticulin fibers were also thick and increased in and around the vascular wall. In the cases of diffuse T cell lymphoma and ATL, the vascular changes did not show any particular pattern. From a morphological view point, the exact correlation between the variously activated biochemical factors and the vascular proliferation in the tissue remains unknown. The present findings suggest the presence of immunological functional activity of neoplastic and reacted lymphocytic cells in the disease groups. Also the vascular reaction pattern might be helpful in the morphological diagnosis of diseased lymph nodes.
