Urinary free light chains (FLC), β_2-microglobulin (β_2-MG), α_1-microglobulin (α_1-MG), N-acetyl-β-D-glucosaminidase (NAG), glycylprolyl dipeptidyl-aminopeptidase (GP-DAP) and albumin levels were measured in the urine from 16 healthy children and 152 patients with various diseases. Compared with healthy children, patients with tubular diseases excreted large amounts of β_2-MG and/or FLC, regardless of their urinary total protein excretion. On the other hand, the increased enzymuria, such as NAG and GP-DAP, observed in patients with tubular diseases was transient. Arginine injection elevated the excretion of various urinary proteins, especially that of β_2-MG. The urinary excretion of FLC was correlated with the degree of the renal scarring in patients with vesicoureteral reflux (VUR). In view of the pathogenesis of the nephropathy in multiple myeloma, the possibility must be entertained that a large amount of urinary FLC is a risk factor in reflux nephropathy.
