OXIDATIVE STRESS IN HEMORRHAGIC SHOCK : PROSPECTIVE CLINICAL STUDY

Lipid peroxides are the biochemical end products of oxidizing reactions with biological membrane-bound polyunsaturated fatty acids. The lipid peroxidation process can be detected by measuring the degradation products of polyunsaturated fatty acids such as malondialdehide. The relationship between the amount of products of oxidative metabolism and the biological antioxidant system determines the outcome in oxidative injury. Methods : In this prospective clinical study, blood samples were drawn from 53 trauma patients (average age 39 years) before anesthesia and surgery (within 1-6 hours of trauma). The samples were used to evaluate their early blood malondialdehide (MDA) and total antioxidant levels. Patients with a shock index>0.9 (SI : Heart rate/Systemic blood pressure) and mean arterial pressure<80 mmHg are defined as being in hemorrhagic shock. Results : Patients were divided into two groups according to whether or not they were in hemorrhagic shock. MDA and total antioxidant status (TAS) were analyzed in each of the groups. Blood level of MDA was significantly higher in patients with hemorrhagic shock than in patients without shock (p<0.05). However, TAS levels were not significantly different between the two groups. When the patient survival status of the two groups was compared, there was no statistical difference in either MDA or TAS levels between them. Conclusion : As a result, early blood MDA level is a factor that determines hemorrhagic shock in trauma patients, but it is not a factor that determines mortality. This study also reports that the antioxidant system is not sufficiently active against free radical damage during the early period of shock.
