In order to examine the effects of normothermic whole body rinse-out (NWBRO) with FC-43 perfluorocarbon emulsion on the course and outcome of entotoxin shock, a number of experiments with male Wistar rats were carried out. These animals, conscious and unrestrained, were infused over 10 min with 60 mg/kg Escherichia coil endotoxin (LD_<100>) and observed for 6 hours. The rats developed hypotension, plasma extravasation, metabolic acidosis, hyperlactemia, hypoglycemia, elevated oxygen extraction from blood, leukopenia, thrombocytopenia, increase in serum GOT and GPT, extensive hemorrhages in the small intestine ; they died 4.9±0.6 hours potinfusion. Another group of rats was subjected to blood substitution with FC-43 emulsion (artificial blood) 30 min after endotoxin application, until hematocrit dropped to less than 3%, after which 6-9 ml of donor blood was exchange-transfused with the emultion. Although NWBRO caused hypotension, transitory rises in blood lactate and GOT levels, and a fall in arterial oxygen content due to the lower hemoglobin (erythrocyte) level, more importantly : it resulted in 98% endotoxin clearance ; improved blood alkaline reserves ; preserved blood glucose ; trimmed elevated GPT and GOT levels ; benefited leukopenia and restored platelet number ; normalized arterial-mixed venous oxygen gradient and O_2-extraction from the blood (which implies preserved blood circulation status) ; attenuated hemorrhage and epithelial necrosis in the small intestine ; and increased the survival rate to 100% at 6 h, 46% at 12 h, and 35% at 24 h. Survival rates were studied in additional groups of endotoxic rats : those subjected to FC-43 blood substitution only had the worst survival rate ; administration of 30 mg/kg methylprednisolone alone, or in combination with NWBRO did not have any significant effect ; while exchange-transfusion with 8-10 ml donor blood alone resulted in the highest survival rate.
