For cleansing of dentures with soft lining materials, chemical denture cleansing is recommended to avoid mechanical stimulation to the denture lining materials. At the same time, reports has been made that denture cleanser pH deteriorates soft lining materials. Also, different deterioration of soft lining materials was found in clinic, even though the same combination of the soft lining material and the denture cleanser was used. The purpose of this study was to evaluate the effect of intraoral pH on deterioration of soft lining materials. At first, as basic experiment 1, in order to examine whether pH exerts a baneful influence on deterioration of soft lining materials, test pieces of three types, Tissue Conditioner (TC), Visco-gel (VG) and Evatouch (ET) were immersed into five different pH solutions for 28 days. The deterioration effect of pH was confirmed certainly through this basic experiment. Next, as clinical experiment, a palatal plate with the soft lining materials was worn by 36 volunteers selected from 52 students and colleagues by pH paper test, who were divided into three groups of intraoral pH, acid group (pH : 6.1±0.34), neutral group (pH : 6.8±0.27), and alkaline group (pH : 7.4±0.35) by carrying out the follow-up investigations of the changes of intraoral pH over the passage of time in hours. The palatal plate was immersed into simple distilled water, and then two different pH denture cleansers during night time in turn by the 28 days according to the usual usage of dentures. However, as volunteers of this clinical experiment were relatively young adults and had intraoral pH close to neutral (pH 5.83-7.56), a supplemental experiment on the supposition that subjects were old persons wearing dentures whose intraoral pH had a tendency to be strong acid was performed at this point. Surface changes of soft lining materials were assessed by the light microscope, the score of surface deterioration modified from Goll's method, and the depth of crater. The results were as follows : 1) This experiment confirmed that human has a stable individual intraoral pH, which has a characteristics of restoring itself immediately even after ingesting strong acid or alkaline foodstuffs. This fact means that it is possible to measure intraoral pH of patients at arbitrary chairtime, and select the most suitable soft lining for the individual. 2) Acrylic type materials TC and VG were susceptible to pH, and weak acid environment was suitable for TC and VG, although TC was suitable in neutral environment also. Silicone type material ET was not so susceptible to pH as acrylic type ones, but this material seemed to be most suitable in alkaline environment when considering discoloring. 3) Acrylic type materials had compatibility with the weak acid type of denture cleanser, and silicone type material had compatibility with the strong alkaline type of denture cleanser. The above results showed that intraoral pH influenced the deterioration of soft lining materials and that the use of denture cleansers reduced deterioration in all intraoral pH environments. Therefore it is most important to measure intraoral pH of the patient by pH test papers if soft lining materials are required, and to reline dentures by selecting and using the most suitable one of soft lining materials according to the pH test, and then to instruct to use the most suitable denture cleanser for standing long use of soft lining materials.
