Seventy-five maxillary sinuses of 40 patients who had had radical surgery were examined by panoramic and CT scans. The main purpose of the present study is to compare the detectability and appearances of postoperative maxillary cyst (POMC) between panoramic and CT views. Results obtained were as follows : 1. Forty-five cases of POMC were detected in the 75 maxillary sinuses by panoramic views. However, when using CT as a standard, 32 of the POMC cases (71%) were classified as true positive and 13 of the cases (29%) were false positive. No POMC was observed in 30 maxillary sinuses by panoramic views. However, 27 of those maxillary sinuses (90%) showed true negative findings and 3 of the maxillary sinuses were false negative according to the CT standard. 2. Of the 32 cases of POMC, 26 (81%) revealed a well-defined monolocular radiolucency. Twenty of the cases (77%) were also accompanied by a marginal osteosclerosis. Of the 32 cases, 19 (59%) were localized in the maxillary sinus on panoramic views ; POMC occupying the whole of the maxillary sinus were easily detected. 3. Of the 75 maxillary sinuses, 57 (76%) revealed a depression of the anterior wall on CT scan. Of the depressions, 40 (70%) showed a right-angled triangle shape of the maxillary sinus on panoramic views. It was strongly suspected that this triangle was caused by the depression of the maxillary sinus anterior wall. 4. New bone development was recognized in 55 (73%) of the 75 maxillary sinuses strictly by CT scans. The new bone development appeared as a higher density in the maxillary sinus rather than in the muscle. The new bone development most often occurred in 12 posterior walls (22%) of 55 maxillary sinuses. New bone development was seen in the medial and anterior walls of the maxillary sinus, but new bone development sometimes was connected to the posterior walls. Of the 55 maxillary sinuses, 19 (35%) were totally filled with new bone development. 5. Of the 75 maxillary sinuses viewed by CT scan, 57 (76%) revealed an osteosclerosis of the cases of the posterior wall. Of the 57 cases of maxillary sinus, 26 (46%) were also accompanied by an unclear and thickened panoramic innominate line. Of the 57 cases, 15 showed an osteosclerosis of the posterior wall accompanied by a well-defined and thickened panoramic innominate line. It was strongly suspected that the thickening of the panoramic innominate line might be caused by the osteosclerosis of the maxillary sinus posterior wall.
